Position Summary

The PHP Social Worker is primarily responsible for conducting comprehensive intake assessments, planning, and implementation of psychosocial services; performing discharge planning; and assisting with care coordination and case management for patients within the adolescent partial hospital program. The social worker collaborates with other members of the multi-disciplinary team including the patient and family to develop treatment goals and assess performance to goals. The social worker may also participate in leading program groups.

*Licensed in state of Minnesota
*BLS

Education:
*Master's degree in social work required

Required Experience:
*Two years of experience as a licensed social worker or similar experience in a health care-related setting

Preferred Experience:
*Two years of experience in a pediatric-related setting
*Two years or more experience in a child or adolescent partial hospitalization program or inpatient setting
*Two years or more experience with individuals with serious and persistent mental illness

Required Knowledge/Skills/Abilities:
*Strong clinical skills with ability to exercise independent judgment, work autonomously, and exhibit high degree of professionalism
*Education and experience providing gender, culture, and age appropriate care
*Excellent group facilitation skills
*Detail-oriented
*Strong computer/electronic medical record skills
*Strong interpersonal skills and ability to work collaboratively
*Excellent verbal and written communication skills

Preferred Knowledge/Skills/Abilities:
*Strong understanding of child development, normal and atypical
*Ability to develop rapport with wide variety of patients and families
*Experience in crisis intervention
